MFC of RSS changes for Intel ix driver

MFC all the outstanding RSS changes, which also includes other. fixes too

Hi guys we're about the rollout a new DC which makes heavy use of Intel XL710 on 10.2-RELEASE +fixes, so I was comparing HEAD ixl with stable/10 to see what I need and found there are few commits flagged as MFC which haven't been processed yet.

I did r277082 this morning as that was straight forward but this one brought in RSS support which isn't present in 10 (there are no plans to MFC RSS support) so I've stripped out the none RSS fixes from this revision.

Given the changes I wanted to run this by you guys to ensure all looks good, in particular what would you like to do with the driver version number?

FYI other revs which look like they need an MFC are: 279033, 279858, 279860 and 284049

I have patches for all the others locally too, but this is the big blocker due to the differences with the HEAD version. The others, listed above, only require minor adjustments here and there.

Switched to removal of opt_rss.h instead of removal of RSS blocks.

This will make future MFC's easier as discussed with sbruno and kbowling.
