rS309726: hyperv/storvsc: Fix the SCSI disk attachment issue.

Summary

On windows 2012R2, if I attach a scsi disk from slot 8, the VM fail to attach it because Hyper-V host did not set SRB_STATUS flag, in addition, it did not fill correct value in INQUIRY response version and format field. As a result, FreeBSD ignores this disk attachment.
