Using the link @kbowling provided, and Wikipedia article on parallel scsi, and googling the individual adapters, I think this is the most accurate.

Sorry to waste everyone's time with decades old legacy equipment, but since the information isn't easy to come by, I think the reference manual is a good place for it. It's also out of the way of other information, and doesn't take up any additional lines.
