rP416170: Update intel-nvmupdate to 5.04
Summary

The new NVM package version is out, updating port to use it.
Changes are limited to distinfo, DISTVERSION and pkg-plist.

Test Plan

Updated NVM on XL710-QDA2 card from 5.02 on FreeBSD-10.3-RELEASE with ixl-1.4.27 installed using net/intel-ixl-kmod, rebooted, checked basic connectivity, ran FTP traffic.
