bhyve nvme: Fix Active Firmware Info

Summary:
Currently Active Firmware Info is not initialized.

Fix is to initialize the Active Firmware Info to Slot 1.

Test Plan: Within FreeBSD/Linux guests, show the Firmware Logpage to confirm.
