committers-ports.dot: Bring file up to date.

Summary

The file was very outdated. Many committers are actually alumni.

This one has been trickier since some people (notably steve@) had its commit bit taken in and restored a couple of times. In those cases, the last removal date is used and the earliest permission granted date is used.

Test Plan

Apply patch and run

dot -T png -o file.png committers-ports.dot
