Add the SCSI Solid State Media Log page (0x11) definition.

sys/cam/scsi/scsi_all.h:
	Add the SCSI Solid State Media log page (0x11) structure
	definition.  This gives the percentage used (in terms of
	lifetime flash wear) of an SSD.
