Use M_SIZE() instead of hand-crafted (and mostly correct) NFSMSIZ() macro
in the NFS server; garbage collect now-unused NFSMSIZ() and M_HASCL()
macros. Also garbage collect now-unused versions in headers for the
removed previous NFS client and server.
