xen: Update packages to install

The current Xen[tm] version is 4.18, and there's only one set of
packages to install. Update the instructions on what to install to be
current. FreeBSD 11 and 12 aren't supported anymore, and 4.18 support
FreeBSD 13, 14 and 15.
