Add QLogic vendor id and device entry for "QLogic NetXtreme II BCM57840 4x10GbE" to bxe(4) driver

Summary

One user reported that the card with the following info is not recognized by FreeBSD:
none145@pci0:4:0:0: class=0x020000 card=0x00ec1590 chip=0x16a11077 rev=0x11 hdr=0x00

I looked into linux source code and this card seems should be supported by our bxe(4) driver.

Test Plan

The user reported that card works with such patch.
