MFde: Start to bring the German version of the dfp-primer to 2015:

• add new chapters/remove 1 unused chapter/rename 1 chapter
• fix Makefile
• link to english versions if a german version does not exist yet.
• Update multiple chapters so that the fdp-primer can be built.

More updates will follow.
