Update Question 4.4.1:

• Replace /dev/ttyv0 with /dev/console in the examples
• Remove /etc/rc.i386 and add a note about rc.conf(5) settings
• Add some examples from the kbdcontrol(1) man page
• Add links for kbdcontrol(1), kbdmux(4)
• Reword a few things (suggested by trhodes)
